Join early access All coming nextWhat it is
Incident Management is a coming-next humblx module for reporting, investigating and closing non-clinical incidents. It will cover slips and falls, property damage, security events, utility failures, staff injuries and near-misses, plus patient-experience incidents such as lost belongings or a serious service failure, including those that start as negative feedback or a complaint. Each incident will keep one record from the first report to verified closure: what happened, where and when, who was involved, photos, the investigation, the root cause, and the corrective and preventive actions. Clinical incidents will stay with your quality process. If one is reported in humblx, it will be routed to the quality team rather than handled in this module.
The challenge
A visitor slips on a wet corridor, a lift stops between floors, a staff member strains their back moving furniture, or a laptop goes missing from a ward office. Today these are reported on a phone call, a WhatsApp message or an incident register few people fill in. Investigations depend on who remembers what, corrective actions are agreed but not followed up, and the safety committee sees a partial picture months later.
The humblx solution
humblx Incident Management will give every operational, safety and patient-experience incident one record, from the first report to verified closure. Staff will report by scanning a QR code, in the app or on WhatsApp, with photos and location attached. Each incident will be classified by type and severity, routed to its owner, investigated with a structured root cause analysis, and closed only when corrective and preventive actions are verified. Negative patient feedback or a serious complaint will be able to become a patient-experience incident with the same RCA and CAPA trail. Clinical incidents will be routed to the quality team for handling under the hospital's own process.
Key features
Reporting by QR, app or WhatsApp
Report from where it happened, with photos, location, time and people involved.
Severity-based routing
Type and severity will decide who is told and how fast, with escalation if nobody acknowledges.
Investigation and RCA
Timeline, statements, photos and a structured root cause analysis on one record.
CAPA with verification
Corrective and preventive actions will carry owners, due dates and closure evidence.
How it works
- 01
Report
Staff will scan the QR code at the location, open the app or send a WhatsApp message. Type, location, time and photos will be captured on the spot. A negative feedback response or complaint will also be able to become an incident.
- 02
Classify
The reporter or duty supervisor will set the type and severity. Clinical incidents will go to the quality team.
- 03
Notify and contain
Owners will be notified according to severity. First actions, such as cordoning off a wet floor or isolating a water line, will be logged with a time and photo.
- 04
Investigate
The owner will record the timeline, statements and evidence, then complete a root cause analysis using a structured method such as five whys.
- 05
Act
Corrective and preventive actions will get owners and due dates. Facility faults will become Facility Maintenance work orders.
- 06
Verify and close
The incident will close only when every action is complete and checked, with evidence attached.
- 07
Review
Trends by type, location, severity and shift will go to the safety committee and the quality team.
What it is built to change
- 01
Incidents stop disappearing
Every report will have a reference, an owner and a status, so nothing will end with a phone call.
- 02
Repeat problems become visible
Trends by location, type and shift will show where the same incident keeps happening.
- 03
Actions that get closed
CAPA items will be tracked until verified, instead of being agreed in a meeting and forgotten.
- 04
A complete record for safety reviews
Safety committees and quality teams will review full incident histories, not partial registers.
Where hospitals will use it
- Slips, trips and falls in corridors, washrooms, lobbies and car parks
- Utility failures: power outages, water supply cuts and lift entrapments
- Security events and property damage: theft, aggression, vandalism, unauthorised access
- Staff injuries and near-misses in housekeeping, engineering and support work
Who uses it
- All staffWill report incidents and near-misses from where they happen.
- Duty managers and floor supervisorsFirst response and containment.
- Safety officer and facility headInvestigation, root cause analysis and CAPA.
- Quality teamWill receive clinical incidents routed from humblx, review patient-experience incidents and check CAPA.
- COO and safety committeeTrends, repeat incidents and overdue actions.
How it connects to the live modules
It will run on the same locations, QR codes, staff, SLAs and dashboards your teams already use.
- Feedback ManagementLiveNegative feedback or a serious complaint will be able to become a patient-experience incident, investigated with root cause analysis and closed with CAPA. Incidents and complaints will record CAPA the same way, so the quality team will work with one format.
- Facility MaintenanceLiveFaults found during an investigation will become work orders with SLAs. A utility failure incident will link to its breakdown work order and the asset's service history.
- Desk ManagementLiveYou will be able to escalate a ticket that turns out to be a safety issue to an incident without anyone re-entering the details.
- HousekeepingLiveYou will be able to add preventive actions, such as extra wet-floor checks in a lobby, to housekeeping checklists and verify them by QR scan and photo.
What we’ll measure
The measures this module is being built to report. We publish results only for modules that are live.
- Incidents reported, by type, severity, location and shift
- Near-misses reported compared with incidents, as a sign of reporting culture
- Time from the incident to the first report
- Time to acknowledge and time to close, by severity
- CAPA actions open, overdue and verified
- Repeat incidents of the same type or at the same location
